1. HOME SERVICE
I.1 USE OF HOME SERVICE FEATURES
Home Services is part of Services provided directly by PT Prodia Widyahusada Tbk (“Prodia”) by providing a chance for you to take blood at a location that you can determine and subject to the Terms and Conditions that apply at Prodia.
I.2 HOME SERVICE TERMS AND CONDITIONS
Here are the terms and conditions between You and the Officer. By using this Service, and/or by registering yourself in the Service, you signify that you agree to terms of use, including that you agree to provide information related to your personal data.
I.2.1 General Home Service Conditions
The Home Service feature can appear in accordance with the terms and conditions set by your access Home Service, including but not limited to types of tests, location, and minimum order requirements. Home Services service charges apply in the form of Home Service Fees.
I.2.2 Provision Changes to Home Service Schedule

If we know or have sufficient reason to suspect that you have to do action immoral, violation, crime or other acts that are contrary to these Terms of Use and/or regulations applicable laws, whether referred to in these Terms of Use or not, then We have the right to and can freeze your Account, either temporarily or permanently, or stop your access to the Application, including Services, Content Party Third, the Offer and/or Payment Method contained therein, carries out the examination, demanding change loss, reported to party authorized and/or take other actions that We deem necessary, including action law criminal nor civil.

Use of Home Services needs ability basic internet usage and skills security general. You are expected to have the necessary internet usage skills and knowledge to utilize Home Services.
I.3 EXPLANATION AND AGREEMENT FOR USE OF HOME SERVICES

Home Services are carried out on a pre-selected schedule and arrangements timetable is carried out systematically. You are obliged to prepare yourself for the Home Service process according to the schedule that has been selected.

Every Home Service will a service fee is charged, the amount of which may change at any time. Service fee Home Service will be charged to you based on the number of Home Service visits. If the examination you have chosen requires you to take blood 2 (two) times in a period certain, then the service fee will be charged to you 2 (two) times.
Officers have an obligation to carry out all Home Services ordered to them. The officer has the right to leave the location taking your blood to get to the location next if you are not ready to accept service and or not providing a response to the selected schedule.
By deciding to use the service Home Service, you acknowledge and agree that you realize these limitations and agree to assume all over risk of this limitation.
