Vaccination Terms and Conditions
These Terms and Conditions ("Terms and Conditions") are the conditions of access for you as a user in using the service vaccination on the U By Prodia application (“APPLICATION”) which is owned and managed by PT Prodia Digital Indonesia (“PRDI/WE”) which is binding between PRDI and You as You (“YOU”). Please read and follow these Terms and Conditions carefully to understand how the vaccination service works in the Application.
1. Definitions in the Terms and Conditions refer to matter as follows:
a. The U by Prodia application is an application that can be downloaded via telephone mobile and/or device electronic (including Android and iOS versions) and provides various service health checks - up, consultations, provider vaccines, health shops, health programs, and information related to healthy lifestyle.
b. Vaccination is entering a vaccine into the body so that it has immunity to one virus/ bacteria or several at once.
c. Medical Personnel and/or Health Personnel are professionals who have the right to provide health services and have permits to carry out their profession in accordance with applicable laws and regulations.
d. Force Major is a situation that occurs beyond the ability of the Parties to overcome including but not limited to natural disasters (floods, earthquakes, hurricanes), fire, riots, and external theft unless otherwise stated in the terms and conditions of this.
2. Objective of Vaccination
a. Vaccination aims to protect somebody from disease certain at a time lower prevalence the occurrence of a disease is expected eradication the disease may occur.
b. From time to time there may be updates to the vaccination service requirements. We urge you to update them periodically from time to time with the aim of getting the quality of the Service regularly improved.
3. Procedure of Vaccination
A. Types of Vaccination
Vaccinations are given to you in accordance with good procedures and rules so that they are safe and effective for You as well as for medical personnel (in this case a doctor) who do vaccinations, as well subject to the Terms and Conditions applicable to Prodia.
Vaccinations can be given to children or mature depending on the type of the vaccine. Currently, the Clinic partner provides vaccination (“Vaccine Provider”) as follows:
1. Hepatitis A
2. Hepatitis B
3. Human Papilloma Virus (HPV)
4. PCV-13 Pneumonia
5. Quadrivalent Influenza
B. Terms and Conditions Vaccination Booking
a. Booking vaccine can only be done 2 (two) days before the appointment date and no later than 30 (thirty) days before the appointment date.
b. You are required to carry out the charging process questionnaire regarding requirements for implementation vaccination via the application before the ordering process of vaccination.
c. If the filling results in your questionnaire regarding requirements implementation vaccination declared “Not Compliant Requirements or Not Recommended”, you will be recommended to carry out a consultation process with a doctor. If you continue the transaction without a consultation with a doctor who allows Your vaccination may be refused at the Prodia clinic, then you must comply with all terms and conditions that apply at Prodia including cancellation of vaccination without refund.
C. Terms and Conditions of Vaccination Reschedule
a. Reschedule is caused by existing constraints from you and/or Provider Vaccines with the following criteria:
1. Incidents are a major force that results in the vaccination process. Force Major or circumstances forcing as a natural disaster, flood, earthquake, riots, or massive withdrawal of funds, which is a direct result of delays and/or non-implementation vaccination process.
2. Unavoidable reasons (illness/accident, immediate family illness, interests that cannot be represented by other people),
b. The rescheduling process can only be done in 2 (two) days before the predetermined schedule, 1 (one) time during schedules are available within the period time maximum 14 (fourteen) days from the predetermined schedule.
c. If you are absent from the specified schedule, with or without existing information, the transaction will be considered scorched.
D. Terms and Conditions of Vaccination Cancellation
a. Cancellation of vaccination with a refund due to the problems that arise from you is not allowed. If you do not fulfill the condition of vaccination upon arrival at the branch, you will be directed to change the schedule in term time 14 (fourteen) days, or 2 (two) weeks counted since the order was paid according to the rescheduled terms and conditions.
b. Cancellation of vaccination with a refund caused by the originating problem from the Vaccine Provider can be done if you refuse to have done vaccination rescheduling, criteria constraint allowed refund as follows:
i. Emptiness vaccination
ii. Absence of doctor on-site vaccinations that are announced suddenly for unavoidable reasons (illness/accident, immediate family illness, interests that cannot be represented by other people) and You do not wish to be carried out rescheduling.
iii. The incident of force major that results in vaccination. Force Major or circumstances forcing as a natural disaster, flood, earthquake, riots, or massive withdrawal of funds, which is a direct result happen delays and/or non-implementation vaccination process.
E. Vaccination Payment
a. You understand and agree that the prices stated in the application of Vaccination may change if you receive other additional services.
b. The application may use third-party services to provide payment methods. Payment methods from third parties may very well have their own terms and conditions which are different from those of the Application and therefore all matters relating to the level of security and guarantee of data confidentiality as well as all forms of related system failures caused by things beyond the control of the relevant Application with payment services provided by third parties is outside the Application's responsibility.
4. Users Agreement in Use Vaccination
a. By using the Application Service, you understand and agree that medical personnel and health workers will provide vaccination services to you according to the conditions you experience and need.
b. You have the right to disagree with the services provided, so you may refuse to receive services on the Application.
c. You understand and agree that if you get services on the Application, your medical record data will be stored online in a system managed by the Application. Data can be accessed by medical personnel and healthcare workers when Users use Prodia Services for the purpose of making appropriate diagnoses, medical treatment, and management decisions for Users according to existing medical science.
5. Use of Vaccination
a. You understand and agree that if you use the Vaccination, then you have agreed to all the Application Terms & Conditions.
b. You understand and agree that before receiving the Vaccination, you must complete the vaccination and self-assessment booklet digitally.
c. You understand and agree that vaccinations are not services that you can perform at your own request. You can request this service, but the determination of the service is determined by a qualified doctor who assesses your medical condition.
d. You understand and agree that before carrying out vaccination, medical personnel or health workers will conduct an interview (anamnesis) with you to ensure that you have no contraindications to the desired vaccination. You have full responsibility for providing personal information accurately and truthfully to ensure appropriateness and accuracy of treatment. If you provide information that is incorrect, the Application is not responsible for any consequences that may occur in connection with providing you with the incorrect information.
e. You understand and agree that if you are under 18 years old, you need to consult a pediatrician before vaccinating.
f. You understand and agree that before vaccinating, medical personnel and health workers the Application will show you the vaccine that will be given to you, vaccine information such as vaccine brand, expiration date, and vaccine quality. This aims to let you know that the vaccine used is truly safe, high quality, genuine, and has not expired.
g. You understand and agree that vaccination is a safe procedure and is carried out for primary prevention of a specific disease but does not rule out the possibility of AEFI (Adverse Event of Special Interest). You understand and agree that during vaccination, medical personnel or Application health workers will explain AEFI specific to the vaccine you received, apart from that you can also access information about AEFI, such as the definition, characteristics, and how to handle it digitally.
h. You understand and agree that medical personnel or Application health personnel will contact you 30 (thirty) minutes after the Vaccination Service to monitor and evaluate AEFI.
i. You understand and agree that if anything unusual happens to your body after vaccination, you need to immediately consult a doctor. However, if something natural feels heavy, sudden, and disturbing, then you are advised to immediately seek medical assistance at the nearest health facility.
j. You understand and agree that the most severe form of AEFI is anaphylactic shock which has the potential to be life-threatening if not treated immediately. This is very rare and if it occurs it is usually within 30 minutes after vaccination. Even though it rarely happens, you are advised to remain vigilant. If this happens, you need to get immediate medical treatment at the nearest health facility's Emergency Room.
k. You understand and agree that our medical staff and health workers will treat you according to medical procedures if your condition changes after you have received the Vaccination Service.
l. You understand and agree that there are other AEFIs that may occur after vaccination, but they are not fully described in these Terms & Conditions. You will get this information from the doctor and the Application knowledge base which can be accessed by you anytime and anywhere digitally.
6. Actions We Deem Necessary
a. If we know or have sufficient reason to suspect that you have done immoral action, violation, crime, or other acts that are contrary to these Terms of Use and/or regulations applicable laws, whether referred to in these Terms of Use or not, then We have the right to and can freeze the Account, either temporarily or permanently, or terminate your access to the Application, including Services, Third Party Content, the Offer and/or Payment Method contained therein, carries out examination, demanding change loss, reported to party authorized and/or take other actions that We deem necessary, including action law criminal nor civil.
b. We will follow up by doing an investigation and/or facilitating Provider The service concerned to report to authorities if we accept Report exists your violation of these Terms of Use or violation to regulation applicable legislation, in connection with:
i. Verbal harassment or violence, including but not limited to, of any physical kind gender, ethnicity, religion, and race, what you do to Provider Service.
ii. Fraudulent
iii. Harassment or violence physical you do to Provider Services; and/or
iv. Harassment or sexual violence, both verbally and physically, what you do to Provider Service.
c. Provisions also apply on the contrary if you experience the action as mentioned above are carried out by the Service Provider towards you.
d. You hold full responsibility to the information provided in the vaccination is accurate. We would not do attempt to validate the information provided by you in the use of Vaccination with respect to content, truth, or usefulness.
e. Use of Vaccination needs ability basic internet usage and skills security general. You are expected to have the necessary internet usage skills and knowledge to utilize it Vaccination.
7. Explanation and Agreement for Use of Vaccination
As is the medical procedure case, there are potential risks associated with the Vaccination. By using Vaccination, you agree to comply Terms and Conditions, Policies Privacy, and risk explained below:
a. Vaccination carried out on the selected schedule and clinic. Arrangement of schedule and clinics carried out systematically. You are obliged to prepare yourself for the Vaccination process according to the schedule and clinic selected.
b. Officer (personnel medical) has an obligation to carry out all over service Ordered vaccinations to him. The officer (personnel medical) reserves the right to refuse service vaccination if found condition inappropriate medical care.
c. By deciding to use the service of this vaccination, you acknowledge and agree that you realize these limitations and agree to assume all over risk of this limitation.
d. You understand and agree that you will provide valid, correct, and reliable information regarding your condition.
e. You understand and agree that you will not carry out threatening actions that have the potential to harm medical personnel, health workers, and other teams of U by Prodia.
f. You understand and agree that the application is not responsible for any medical conditions you experience due to misuse and actions you take that are contrary to medical advice from medical personnel and/or health workers, for example not immediately consulting a doctor or coming to the nearest health facility when an AEFI occurs.
8. Closing
a. If these Terms and Conditions are made in Indonesian and English, then both will have the same binding nature. However, if there is an inconsistency between Indonesian and English, the Indonesian version will apply.
b. By accessing or using the application services, you understand and agree and are bound and subject to all these Terms and Conditions. The relationship between PRDI and you are an independent relationship and does not have any agency, partnership, joint venture, employee-to-company, or owner-to-investor relationship that would be created or created by this Agreement.
c. The headings in these Terms are made only as a reference, and in no way determine, limit, explain or explain what is contained in the article or provision.
